A successful root cause analysis is often a team effort, involving various fields of expertise using a structured work methodology. To be able to effectively remedy a fault so that it does not reoccur, it is usually necessary to know the root cause or causes. Faults or failures can be of a small or larger nature; what they have in common is that they reduce availability in a plant. Systematic work with root cause analyses, during which recurring faults and serious incidents are successively eliminated, will not only increase availability. It will also lower maintenance costs and have a positive impact on the environment, health and safety.
